379th BOMB GROUP WWII ASSOCIATION MEMBERSHIP FORM
(Please use this form for membership application, renewal and update of address,
phone, email, etc.)

Regular membership in the Association is open to all persons
who were members of the 379th Bombardment Group (H) assigned to units attached
to the Group at Kimbolton, Huntingdonshire, England (AAF Station 117) during
World War II. Officers and enlisted men assigned to Kimbolton totaled more than
6000. Spouses, children and their spouses, grandchildren, widows and widowers
of said veterans are also eligible for regular membership and are cordially
invited to join the Association.

All current paid dues members receive three issues annually of CONTRAILS, the
official newsletter of the 379th. Annual dues (for the period Jan 1 through
Dec 31) are $15.00 for everyone; billed in two-year increments ($30.00). Members
may elect to become Life Members by paying a one-time fee from the following
age-based schedule:

21-35 = $270.00	51-55 = $195.00	61-65 = $135.00	71-75 = $80.00
36-50 = $230.00	56-60 = $165.00	66-70 = $105.00	76+ = $75.00
